Required Documents

The following documents are required for the online application process. Please have these documents ready to upload upon filling out the student’s application.

  1. Copy of applicant's certified birth certificate

  2. Report card (most current)

  3. Copy of current immunization records
         a. 5 DTaP, 4 doses meet the requirement if at least one was given on or after the 4th birthday.
        b. 4 Polio, 3 doses meet the requirement if at least one was given on or after the 4th birthday.
        c. 2 MMR, given on or after the 1st birthday.
        d. 3 Hepatitis B
        e. 1 Varicella

  4. Pastoral/mentor/teacher referral form (available on the My WCA page)
      Direct the pastor, mentor, or teacher to our forms page for submission prior to your assessment date.

Jr. K - Additional requirements:

  1. Health Examination for school entry. (dated no earlier than January 1st, 2024)

  2. Student must be four years of age by September 1st, 2024.

Kindergarten - Additional requirements:
